This series has been running since 1997 and shows no sign of stopping, which creates an obvious problem for anyone writing a quiz about it. Cover everything and you produce something only lifelong readers can attempt. So this one stays where the audience actually overlaps: the crew, their individual jobs and fighting styles, the fruit-based power system, the early geography, and the family connections that the story keeps returning to. Fifteen questions, none of them requiring knowledge of the recent arcs. What makes it Medium rather than Easy is that it does not stop at names. The crew of this ship is organised like an actual ship, with each member holding a specific role that gets referred to constantly, and the questions ask about those roles and those fighting techniques rather than just who is who. Anybody who watched the first few hundred episodes has everything they need. Anybody who knows the characters only from images and memes will find the second half harder.

Every member of this ship was recruited to do a specific job, and the story treats those jobs seriously. There is a navigator, a cook, a doctor, a shipwright, a musician and so on, and the recruitment of each one is a whole arc rather than a scene.
That is why role questions are fair game and why they trip people up. You can watch for years, know a character intimately, and never consciously file what their actual function on the ship is.
Eating one of this world's cursed fruits grants a permanent, usually bizarre ability, and takes something significant away in exchange. That trade is a running plot device: characters who cannot swim in a story set almost entirely at sea.
Each fruit has a formal name, and those names follow a repetitive pattern that makes them memorable once you have heard a few. Questions about powers tend to want the fruit's name rather than a description.
Several of the main cast fight using named, formalised disciplines rather than generic combat. One member fights with multiple blades at once in a way that should not be physically possible, another fights exclusively with his legs for reasons rooted in his profession.
These style names are used constantly in dialogue and are among the most quotable things in the series.
This story is unusually interested in brothers, adoptive parents and inherited legacies. Several major emotional beats depend on a relationship revealed long after the characters were introduced, and some of those bonds are sworn rather than biological.
A couple of questions come from here, and they are the ones most likely to separate viewers who followed the story from viewers who followed the characters.
Sitting above all the individual crews is a global authority that classifies pirates, suppresses history and maintains the order the protagonists keep disrupting. Understanding that it exists, and that it is an antagonist rather than a neutral backdrop, covers the world-building questions.
Roles and species are the quiet difficulty. Several crew members are so visually distinctive that viewers never think about what job they hold or what animal they actually are, and both come up.
